Second Ballot Request for the June 7, 2011 San Juan Capistrano Special Municipal Election
One may request a second ballot if the first ballot was spoiled, or sent to an incorrect mailing address. To request a second ballot, please fill out the Second Ballot Declaration, sign it and return it to our office. You can return the Second Ballot Declaration by fax, mail, or by returning it to our office. The deadline to request a second ballot is May 31, 2011.
